The decapitated head is an item used in Creature of Fenkenstrain. It is one of the five parts of the monster that Dr Fenkenstrain is creating. Players can find it by digging a grave where the Gardener Ghost lies.

The head was actually the Gardener Ghost's head, until Dr Fenkenstrain murdered him via decapitation. The head is then used with a pickled brain, which originally belonged to Lord Rologarth, and becomes the decapitated head (brain).
