The Floor Is Spiky is an achievement that requires the player to participate in a successful Zemouregal & Vorkath encounter in hard mode without taking damage from the "Bone Spikes" attack.
Movement abilities will help dodge this attack, which is telegraphed with grey smoke covering a 5-by-5 area. Since there will be one set of Bone Spikes summoned for every player in the arena, it can be very challenging in larger groups due to overlapping spikes.
This achievement fails if any player in the instance gets hit by Bone Spikes.
